LASA 1—Diagnosis for Substance Use Disorder with Co-occurring Disorder

In your professional capacity as a substance abuse counselor, you are not permitted to give diagnoses for mental disorders other than those related to substance abuse. However, you will likely be exposed to clients with mental health disorders in the assessment or treatment process. The competent substance abuse evaluator will be able to identify clients who present mental health disorders and make the appropriate referrals. Providing appropriate referrals for clients is common in the substance abuse field.

In M1: Assignment 3, you developed a case history for a person with an addiction. In this assignment, you will develop a case history and include one mental disorder. The inclusion of a mental disorder in your case study will aid in the examination of co-occurring disorders. You will then present your diagnosis and referrals.

This assignment has two parts. Complete both parts.

Part I

Create a case history for a person with an addiction and a mental disorder. Do not include identifying information. Include the following elements in your case history:

  • Demographics—including age, gender, ethnicity, socioeconomic status, marital status, number and ages of children, living arrangements, and religion if applicable
  • Presenting problem
  • Previous psychiatric treatment of patient
  • Previous chemical treatment of patient
  • Chemical history
  • Medical history—including disabilities if applicable
  • Work and education history
  • Legal or financial concerns
  • Family background
  • Concerned person involvement
  • Referral source

Part II

Analyze the case history and present your diagnosis as related to substance use disorders. Include the following in your diagnosis:

  • Summarize key case study elements you would use to formulate a diagnosis and guide your treatment plan.
  • Identify at least two assessment tools that you would use to formulate a diagnosis. Justify why you have selected these tools.
  • State your diagnosis related to substance use disorders. Justify your reasons for your diagnosis.
  • Identify other concerns and offer recommendations and referral opportunities. Justify your reasons and explanations.

Write a 3–4-page report in Word format. Apply APA standards to citation of sources.

Group polarization can be defined as the process that can occur after group interaction, when an initial group attitude (e.g., “Our company is the best at providing Service B in this community”) becomes more extreme (e.g., “Our company is the best at providing Service B in this entire region”). Group polarization may be particularly likely when the issue at hand is deemed particularly “important” (Crisp & Turner, 2007, p. 169).

Groupthink is considered a more extreme version of group polarization and refers to decreased flexibility in group processing and can, in turn, lead to decreases in group productivity. This has never been more evident than in the space shuttle Challenger disaster in 1986, when Challenger exploded in mid-flight. In this case, evidence suggests that groupthink played a major role in this event.

For this Discussion, review this week’s Learning Resources. Pay particular attention to the impact of groupthink and group polarization. In addition, using the Walden Library, research alternatives to groupthink and group polarization and select an article that discusses an alternative.

With these thoughts in mind:

Post by Day 4 a description of a situation in which you experienced or observed negative consequences resulting from groupthink or group polarization. Then describe two consequences as a result of groupthink or group polarization in your example. Finally, using an article from the current literature, explain how one alternative to groupthink or group polarization could have been used in that situation.

Both environmental and genetic influences impact fetal development. Some substances (e.g., folic acid, hormones, etc.) have the biggest influence at sensitive periods during gestation. Early in gestation, when neurological development is at its peak, folic acid is most important. Hormones, such as androgen and estrogen, are necessary for external genital differentiation between 9 and 12 weeks of gestation. Prenatal alcohol exposure, however, can impact fetal growth at any time during gestation. On the other hand, environmental influences such as maternal genetics, nutrition, health, and immunization can have a positive impact on fetal development, mitigating the impact of substances and other negative influences. In addition to environmental influences, you must also consider the impact of genetic influences. Genes determine not only an individual’s physical features at birth—they also contribute to hormonal processes throughout the lifespan. The interaction of environmental influences and genetic influences impacts the development of a fetus.

For this Discussion, you will examine environmental and genetic influences on fetal development.

To prepare for this Discussion:

  • Select one genetic influence and one environmental influence on fetal development and think about how these influences might impact each other.

By Day 4

Post a brief description of the genetic influence and the environmental influence you selected. Then, explain how the environmental influence might positively or negatively affect the development of a fetus with the genetic influence you selected. Be specific and provide examples. Use your Learning Resources to support your post. Use proper APA format and citations.

By Day 6

Respond to at least one of your colleagues’ posts and include an additional influence (genetic or environmental) that could interact with one of the influences identified by your colleague. Use your Learning Resources and other scholarly sources to support your post.

Equilibrium in Natural System

When pressures and temperatures become unbalanced in Earth’s atmosphere, it can produce some dramatic effects as the system tries to rebalance itself, or re-equilibrate (reach equilibrium). Many of these changes are things that are witnessed every day on a small scale in the form of weather, such as wind, rain, snow, fog, etc.

Sometimes, however, Earth’s atmosphere experiences very large imbalances which can cause massive and very disruptive change. The extreme weather events of 2011 are good examples of extreme changes in pressure and temperature that required re-balancing (equilibration). Not only did they have environmental impacts, but also disastrous consequences for the hundreds and thousands of people in their wake.

Then, during the week, discuss with your classmates:

  1. What kinds of natural imbalances might affect your region? What makes your region particularly vulnerable to these imbalances?
  2. Provide a specific example of a weather incident/natural disaster that occurred in your area in the past and discuss how the incident was handled by local residents and town/state officials.

Comparing Three Theories

For this week’s discussion you will consider the scenario below and respond in detail to each of the questions below. Here is the scenario you will use:

Roberta has come to counseling because she is contemplating a divorce from her husband of 12 years. She has been unhappy in her marriage for several years and doesn’t think that she and her husband can rekindle the interest necessary in their relationship to make it work. To complicate the situation, she has been seeing a married co-worker for several months on the sly and is sexually involved. Roberta thinks she loves the man she is having an affair with and would like to marry him. Though he wants to continue seeing her he has made it clear that he is unwilling to end his marriage or break up his family to be with her. Roberta and her husband have three young children and she knows a divorce would be crushing to the children. She’s come to you for counseling to sort through these issues.

1. In your opinion, which of the following counseling theories would you prefer to use to try and help her: Person-centered, Gestalt or Reality? Choose only one and explain your reasoning in detail.

2. Using your chosen theory from question 1, would you go about helping her sort through her confusion? Be sure to bring in concepts from your chosen counseling theory to show how you would specifically work with her.

3. Roberta is looking for answers to questions she feels unable to answer herself because she is so emotionally involved. After she tells you her story she asks you to give your perspective and advice on what she should do. How would you respond to this request? What potential problems might arise if you do give her direct advice? What potential problems might arise if you don’t give your perspective? Be specific and detailed in your response.  What would your chosen counseling theory have to say about giving direct advice?

4. At the end of the session, Roberta is very distraught and asks if you would be available to meet her for coffee later that day to continue the conversation in a more casual environment. Under what circumstances might this be appropriate, if at all? What potential conflicts might this create? Be specific and draw upon information from your text and lecture materials to support your points.

(J Saunder) In my opinion, the best counseling theory that would help Roberta is the person- centered theory. Person- centered therapy is a more comfortable setting for Roberta and her husband to try and rekindle their marriage.  This is also the idea environment for Roberta to address the affair that she has been secretly having without feeling like she is being judged or reprimanded.  The person-centered theory has three therapeutic conditions for change to occur in the client (Argosy University, 2017).  The three conditions are: Counselor congruence, Empathic understanding, and Unconditional positive regard (Argosy University, 2017).  The condition of counselor congruence means that a counselor will be honest with the client.  The counselor has experienced what is said to the client (Argosy University, 2017).  The condition of empathic understanding is knowing what the client is thinking, feeling, and experiencing, then communicating it to them (Argosy University, 2017).  The condition of unconditional positive regard is to respect and accept the client despite their wrong-doing (Argosy University, 2017).  The ultimate goal of this therapy is for the client to reach self- actualization (Argosy University, 2017). 

I would go about helping her by making her feel comfortable enough to allow herself to think clearly and logically about her decision.  I would utilize listening skills to allow her to convey her feelings without interruption.  I would pay close attention to her verbal and nonverbal messages (Argosy University, 2017).  I would also explain Maslow’s Hierarchy of Needs pyramid.  I would then ask her to identify what she felt was her place on the pyramid, afterwards, I would identify her goal placing which would be self-actualization (Argosy University, 2017).   I would ensure that the sessions that we had were centered around the changes within herself that would result in our goal being met (Argosy University, 2017).  These changes include: becoming open to experience without distorting it to fit into already established belief systems, trusting her own judgment, utilizing her inner source of evaluation, and becoming willing to continue to grow with the understanding that she can always be in the process of becoming better (Argosy University, 2017). 

            I would respond by saying that I would be more than happy to offer my direct professional advice which would be similar to my personal advice.  The potential problems that might arise if I give her direct advice would be to violate the code of ethics set by the ACA stating that counselors will not impose their feelings, thoughts, or views on the client.  A potential problem if I decline to give direct advice would be that I potentially loose her as a client because she is dissatisfied with the decision.  The person- centered therapy says that a therapist does not manipulate the change in the client but creates the conditions that is needed for the growth of the client (Carl Rogers and Person-Centered Counseling, 2014).

            I would respectfully decline her offer and I would instead give her my office hours.  This would not be appropriate at any time because of the code of ethics from the ACA.  With her being my client, I am prohibited from violating the boundaries between client and counselor ( American Counseling Association, 2014).  The conflicts that this create is that it could be seen as me creating a personal relationship with a client, accepting gifts if she decides to buy, and the assumption that I would accept such an offer from all of my clients.  The point of person-centered therapy is that the client has the most comfortable conditions possible for their growth (Argosy University, 2017).

Evidence Based research informs social work in several different aspects. For example, evidenced based practice because it provides guidelines for how certain applications in the process of social work. As discussed in our weekly resources, some evidence based practices, display how certain codes and ethics could be more beneficial in practicing and applying social work, (Gossett & Weidman, 2007).Another positive to evidence based practice research, is that it expresses validity tons of research discussing the same concerns. This could be beneficial to social work because it could allow social workers to develop ideas and plans around evidences based work that have came to the same consensus regarding certain social work issues in society. This is some reasons why evidenced based practice and research is essential to social work applications.

A Class Divided explores the nature of prejudice. Third grade teacher Jane Elliott deliberately created a classroom situation to teach her students how it feels to be on the receiving end of discrimination. This is an encore presentation of the classic documentary on third-grade teacher Jane Elliott’s “blue eyes/brown eyes” exercise, originally conducted in the days following the assassination of Rev. Martin Luther King Jr. in 1968. This classic classroom experiment, conducted in the late 1960s in a small Midwestern town, demonstrates how quickly and easily schoolchildren can internalize prejudice and discriminate. Years later, these children discuss the valuable lessons they learned from this experiment. Elliott employs this same teaching strategy with a group of adults in the workplace, and discusses their reactions.

This assignment is designed to allow you to synthesize what you have been learning about the various dimensions of diversity and the necessity of treating everyone in an inclusive, sensitive and respectful manner.

Discrimination Paper Part I: Interpersonal Reflection

After viewing the video, consider the following questions and write your response. Your response should be 4-5 pages and should address each question thoroughly, reflecting an accurate representation of what you have learned in this course. Demonstrate scholarship by utilizing supporting resources to justify your ideas and responses:

  • What did you learn from the film? What scene or scenes do you think you’ll still remember a month from now and why those scenes?
  • Did any part of the film surprise you? Do you think someone with a disability, of a different sexual orientation, an older American or some of a different religion would also find it surprising? Why or why not?
  • Both Elliott and her former students talk about whether this exercise should be done with all children. What do you think?   • How can negative and positive labels placed on a group become self-fulfilling prophecies? Use an example from what you have learned about people with disabilities, older people, sexual minorities, or people of differing religions.
  • Based on what you have learned in this course, discuss an example (either from the video or from your experiences) that illustrates each of the following statements:
    • Dimensions of diversity may be hidden or visible.
    • Dimensions of diversity are in a constant state of flux.
    • Dimensions of diversity are not always clear-cut or easily defined.

Discrimination Paper Part II: Personal Interview

For Part II of this assignment, you will have a conversation with someone who you feel may have faced discrimination. Examples include someone with a disability, an older American, someone who is a sexual minority, or someone who lives in a multicultural family. After choosing an individual to interview, explain to this individual what you have seen in the Class Divided program. Invite them to watch the program, or parts of the program, with you. After watching or discussing the program, pose the following questions to the individual. Be sure to explain the reason for your questions and why you have selected them to participate in the interview:

  • What, if any, discrimination do you experience?
  • How have you coped with this situation?
  • What do you think needs to change at the cultural level to reduce discrimination?

Following your refection (Part I listed above), add 2-3 pages to your paper which addresses the following:

  • A description of the individual you chose to interview and why. Explain how you went about approaching this individual for the interview.
  • What are your observations about the person’s view of discrimination and how it affects his/her daily life?
  • Did the interaction with the person change your view of discrimination? If so, explain how the interaction has affected you either positively or negatively. If it did not change your view of discrimination, explain why.
  • How well do you think you would cope with discrimination from this person’s perspective?
  • Finally, what is the best manner in which to advocate for those facing discrimination? What actions will you change based on what you have learned in this course and how will you serve as an advocate for those individuals who face discrimination?

Your final assignment, consisting of both Part I and II, should be approximately 6 -8 pages. Be sure to address each topic listed above and, as appropriate, cite the online course, the textbook, and other credible sources to substantiate the points you are making. For example, when discussing an example of how diversity may be hidden or invisible cite sources, which you have referenced to substantiate the points you are making.
