When nurses go to work, their patients should be their priority. The nurse should be focused on patient care, doing no harm to their patient and attempt to use critical thinking and interventions in attempts to spend their 12-hour shift to improve the wellbeing of the patient. We should be asking ourselves, how can I improve the outcome of my patient today?

 

Unfortunately, nurses today are placed under tremendous pressure to accomplish more in less time, with less staff and less resources. The days of being able to spend quality time with your patient and addressing needs is cut short. Patients and families notice the nursing shortage as well, nurses are often too busy to talk to their patients/families, address psychological and education needs (Poulton, 2013). With staff cuts/shortage the impact of service is effected, the quality of care, and increase in medical errors, etc. This not only affects hospitalized patients, but also care in outpatient clinics and other health care services.

 

There have been many studies which show when nurse to patient ratios are increased and nurse burn out happens, patients die, increase in injuries, increase infections, and discharged too soon with improper patient education (Truth about nursing, n.d.).

 

 AACN expanded the nation’s centralized application service for RN programs, the primary reasons for launching NursingCAS was to make sure that all empty seats in nursing schools are filled to better meet the demand. In 2016, more than 38,800 vacant seats were identified in baccalaureate and graduate nursing programs. NursingCAS provides a way to fill these seats and maximize the educational capacity of schools of nursing (AACN, 2017).

The bureaucratic agency I have chosen was the United States Environmental Protection Agency (EPA). The main mission of EPA is to protect human health and the environment. Some of the EPA’s goals are, but not limited to:  Americans have clean air, land and water; National efforts to reduce environmental risks are based on the best available scientific information; Contaminated lands and toxic sites are cleaned up by potentially responsible parties and revitalized (United States Environmental Protection Agency, 2018).

           The EPA has many special interest groups involved with the agency. The Office of Administration and Resources Management (OARM) provides national leadership, policy, and management of many essential support functions for the Agency, including human resources management, acquisition activities, grants management, and management and protection of EPA’s facilities and other critical assets nationwide. The Office of Air and Radiation (OAR) develops national programs, policies, and regulations for controlling air pollution and radiation exposure. The Office of Enforcement and Compliance Assurance (OECA) goes after pollution problems that impact American communities through vigorous civil and criminal enforcement. The Office of Environmental Information (OEI) identify and implement innovative information technology and information management solutions that strengthen EPA’s ability to achieve its goals. There are multiple other special interest groups involved with the EPA as well (United States Environmental Protection Agency, 2018).

           The EPA’s website is user friendly, it gives you access to all of the special interest groups, information on all of them and how they play a role in the EPA, as well as contact information and office location, making them all easily accessible.

Errors in medication happen to be among the most common adverse situations in the field of healthcare services. Drug safety is a term that is used to refer to the science involved in collecting, detecting, assessing, monitoring and preventing the adverse impacts of the pharmaceutical products. As a nurse, it is essential to make sure that drug safety is enhanced at all time. One approach that is used in making sure that this happens is by enhancing the practice environment.

One of the ways of applying this drug safety approach is by offering a supportive practice environment. In this case, there is the need for enhanced teamwork between the personnel who are involved. They entail the nurses, pharmacists and the physicians (Jeffs et al., 2013). In addition to that, there is the need for enhanced opportunities for the nurses to take part in decision-making. This is a strategy that is aimed at making sure that the various decisions that are arrived at continue enhancing the safety of the patients at all times.

It is also essential to have continued education opportunities for the members of the profession. In this case, it is normally seen that as time goes by, there is the need to enhance the skills that a person originally had before. A nurse is not needed to be involved in activities such as the administering of a drug which he or she does not seem to be familiar with. Accurate as well as the current drug information needs to be availed to all caregivers within a healthcare setting.

Adequate information needs to be enhanced within a healthcare setting. For this case, most of the drug safety issues are as a result of miscommunication among the various healthcare providers. As a means of countering this challenge, there is the need to enhance effective communication between the team members (Boysen, 2013). A common method that is used in this scenario is the SBAR (situation, background, assessment, and recommendations). The impact of the use of the improved practice environment approach is that it goes a long way in eliminating the various instances of errors. This means that the various consequences that arise from these errors are also minimized.

Being a change agent is a nurse that brings about change (positive change) into the work environment and helps facilitate the change.  They facilitate the change by helping bring about the change in a positive manner and show why the change is necessary and what improvements it will make in their day-to-day work. I have recently been a change agent in the implementation of a new labeling process for our specimen and blood collection process.  We needed to change the way we labeled our specimens to help facilitate the way they were processed at the core lab.  This was going to impact our work flow in the clinic dramatically though.  First, I talked with the staff about the upcoming change (which was going to cause a slight increase in work for my staff).  Then, to show the impact of the change to our staff I took them to the core lab where our specimens are processed and showed them how much extra work goes into the current process vs. the new process.  After seeing the decrease in work that it causes the core lab our staff was willing to take on the slight increase in work to help them out. Another example of being a change agent was when we implemented LEAN into our work flow several years ago.  This was a process that done over a whole year, but over this year we had to introduce new process to the staff and help them see the LEAN way of trying things.  We would get staff impute when we were drafting the process changes to help with the understand of the why the changes were important and needed to be done.  With LEAN one big development was the development of standard work so that all patients were to receive the same standard of care every time they walked through the doors.  We had staff help us with the processes and so when we rolled it out, they understood the importance and we all had the same goals in mind, which was the patient.  I’m sure that we all are change agents everyday with small things we do, but they affect someone or somebody at some point.  Change is hard but we should always remember that change is often for the good.

Evidence-based practice projects are fundamental in medical practice. This is the reason as to why such projects emanate into results that are statistically significant. A distinction between clinical significance and statistical significance will be discussed in its relation to evidence based projects (Gillani, 2011). The use of clinical significance in supporting positive outcomes in the project will also be highlighted.

Clinical significance refers to that ability displayed by a treatment to enable a patient return to his or healthy state of body functioning. However, it differs from statistical significance in a sense that it is more objective i.e. it determines whether the prescribed treatment was able to achieve the intended purpose (Sedgwick, 2014). Statistical significance, though a determinant that was only used sometimes back is expressed as a variable meaning that it is never exact on whether the treatment recommended is going to restore a patient’s normality. It operates on the principle of probability.

The ultimate aim of the evidence-based practice project is to bring forth positive outcome. Incorporation of clinical significance can prove to be of great help in achieving this. Carrying out a clinical interpretation in the entire research process will be fundamental in ensuring that the patient’s safety, as well as efficacy need, is put into consideration when it comes to decisions made. Physical activity is very great for the body. With physical activity and exercise the body is able to work to grow stronger. The body systems are able to build in strength as well as helping with immunological problems. As nurses it is important to assess whether or not individuals partake in physical activities. Living a sedentary lifestyle can lead to poor health, increases the chances for chronic problems, and may also lead to mental issues such as depression and anxiety. According to Westcott et. al. (2017), physical exercise can help make “changes in body weight, BMI, percent fat, fat weight, lean weight, WC, HC, W/H ratio, SBP, DBP, TBC, LDL, HDL, TRI, and HbA1c.”

Changes in these things can help with preventing certain diseases. According to Jarvis (2015), physical activity “shows a reduced risk of death from [cardiovascular diseases] CVDs and respiratory illness.” Cardiovascular diseases that could be prevented by physical activity includes high blood pressure, strokes, heart attacks, etc. As nurses it is not only important to assess whether or not the individual is meeting their physical activity needs, but ensuring that they provide teaching and resources to any individuals that may need help when coming to physical activities. Nurses should assess patients to see what kind of activities they like to do and try to plan out resources that the patient can utilize when leaving the health care setting to ensure that they will continue getting physical activity. Nurses can work alongside with physical therapists and social workers to make these patients needs met. This is an important aspect as it can help with patient care and orientation to the importance of physical activity and such. Many patients may refuse these actions as they may feel uncomfortable being physically active while they are sick; however, it must be taught to the patients in order for them to understand how important it is.
