My vision of a healthy community would be a place where people are outdoors, physical activity is high priority.  Fast food establishments were very rare, people love eating at home, preparing food from their gardens.  Smoking was nonexistent.  Drug abuse was unheard of.  People knew their neighbors, waved at them and invited them to block parties,  and could trust them for a strong neighborhood watch. 

 

This is a utopia community.  The word comes from the Greek, it means “no place” (Wikipedia website, 2017).  Utopia does not exist, however, there are communities that provide increased quality of life through the environment that people live in.  In 2014 Washington D.C. was the healthiest city to live in.    The determination is based on an American Fitness Index.  It is data using measures on how well a community serves the recreational and fitness needs of its residents.  A new measure was added in, “the walking score”.  The obesity rates and smoking rates were down in these communities.  Residents rates of heart disease, diabetes, asthma, and general health are also important.  Cities need to prioritize the health of its residents.  The cities culture and politics need to be at work supporting health and lifestyle changes.  As just one example of this, let’s look at parks. First place winner Washington spends $398 per resident on its parks as compared with Memphis, last ranked at #50, which spends just $26 per resident (Haiken, 2014).

 

What if your community does not match your ideal?  This gives opportunity to focus on policies that affect communities ‘physical, social, and economic environment.  Rates of asthma have been rising in the population of low-income and people of color disproportionally.  How do policy and community affect asthma?  Studies found a strong correlation between air pollutants and asthma.  Public policies determine air pollution tolerances and how much specifics will be allowed to determine what is air pollution.  Advocates make change, they work locally to educate and inform the public and policy-makers about asthma and the correlation between pollution (Bell & Standish, 2005, p. 340).  We can be agents of change to create the communities we want to live in and raise a family in.

“Successful communication between healthcare providers and their patients from different cultural backgrounds depends on developing awareness of the normative cultural values of patients and how these differ from the cultural values of most western medical professionals. When cultural differences are poorly understood, a variety of adverse clinical outcomes may result: reduced participation in preventive screenings, delayed immunizations, inaccurate histories, use of harmful remedies, non-compliance, and decreased satisfaction with care” (Carteret, 2011).

 

There are many cultural considerations to be aware of while interviewing Sue Li who is of Asian American ethnicity. The Asian community as a whole is conservative no matter the sex of the person. Sue Li should first be asked if she would prefer to speak with a male or female nurse. Since the patient is only 20 years old there may be some resistance in communication. The Asian Community has a strong bond with family. Their family extends to include all relatives not just immediate members. Loyalty is expected within the family, and dishonor or disgrace is not tolerated. Self-control is expected from all family members this demonstrates stamina and strength in crisis. In the healthcare setting Sue Li may be unwilling to show strong emotions, pain, or grief because of cultural values. Cultural beliefs about health and sickness often conflict with Western medicine, which prevent some Asian-Americans from seeking help for symptoms or sticking with treatment.

 

 Another important consideration the nurse must check for is that Sue understands what she is being educated about. There may be a language barrier. Sue Li may give the nurse a gesture as though she understands when she is actually confused about her diagnosis or treatments. It will help if the nurse avoids asking yes or no questions. The nurse must also give the patient time if she wants to confer with her family. Unfortunately this can be time consuming and possibly delay care but building a trusting relationship is essential when dealing with different cultures (Carteret, 2011).

 

The Abuse Assessment Screen (AAS) was created for the detection of abuse in pregnant women, and it is the oldest screening tool used today. It consists from three to five questions, and is also used to increase documentation in medical records. The questions assess physical, sexual, and emotional abuse prior to and during pregnancy. The AAS I usually conducted by the nurse but can be conducted other members of the hospital such as, counselors, physicians, or nutritionists. The tool is even translated into different languages, and it also includes body maps for documentation of injuries (Women’s Health, 2009).

 

If abuse is discovered the nurse should not only report it but offer support to Sue Li. The nurse can provide her with resources to receive help and education. For instance, a nurse could give Sue Li the phone number of a domestic violence hotline. Also, the nurse may show Sue to a phone and an opportunity to make the call. After the nurse assesses Sue’s home environment and determines if it is safe the nurse can refer her to a battered women’s shelter.The National Domestic Violence Hotline, 1-800-799-SAFE, is an excellent source for both patients and nurses. These sources will allow the nurse to give Sue Li additional knowledge, encouragement, and the assurance that she can come back to the hospital for additional help (Carteret, 2011).

my background :   In regards to the professional field, I received my BSN degree in Habana Cuba in 1996 and received my RN license in the State of Florida on October 13, 2014.  I NEED A POSITIVE COMMENT BASED IN THIS ARGUMENT

 

 

Not only is teen pregnancy a high-risk situation, it is frightening for the teen to even consider the possibility that she is pregnant.   Denial lends itself to the high incidence of late prenatal care if there is prenatal care at all.   There were just under a quarter million pregnancies in women 15 to 19 in 2014.   This number went down from the 2013 figure.   Teen pregnancy, although falling in numbers, remains the highest in the industrialized countries (Weiss, 2017) .   For the infants of these mothers, there is a higher rate of low-birth weights.   Low-birth weight is associated with infant mortality and other health problems such as cerebral palsy, mental retardation, dyslexia, and hyperactivity.   These children fall victim to abuse and neglect and tend to suffer in school (Edelman, Mandle, & Kudzma, 2014, p. 525) .  

What increases the chances of teen pregnancy?   Living in poverty, having a mother that gave birth as a teen, living in a single-parent home, living in a home with frequent family conflict, early sexual activity, early use of drugs or alcohol, low self-esteem, and lastly, race and ethnicity are all risk factors for teen pregnancy (“Youth.gov,” n.d.) .

In Arizona, teen pregnancy rates have dropped 34% from 1999 to 2009 (“Teen pregnancy on decline,” 2011) .   There are fewer births overall, even in the Hispanic population.   This is thought to be because of fewer Hispanic population overall in Arizona.
