1. Economic Dynamics of Health Care Delivery Models

After reading Chapter 7 and the required resources for this week, consider the following scenario:

HIV Help-Inc., a non-profit organization focusing on the prevention of HIV/AIDS just received a $10 million grant to fund several projects.  The organization is currently located in an older building that needs extensive repairs. The organization is using outdated office equipment In addition, one additional staff member is needed in order to keep up with incoming phone calls and requests for presentations and community outreach activities. A portion of the grant – $2 million – is allotted for business improvements which can address one of these three areas: repairs to building, outdated office equipment, and more staff. The remaining $8 million is to be used to further enhance the continuum of care level to provide access to preventive health services.

  • Section A: Create a cost benefit analysis for an update that will improve the business: structural, office equipment, or staff. You can be creative in determining what the business needs.  The cost will use the full amount of funds allotted to this improvement ($2 Million), so you can only improve one of these three needs.

  • Section B: Create a cost effective analysis to determine how to best spend the $8 million portion of the grant funding on education and other preventive services.

The organization currently serves the community and clientele with community education classes at schools and community centers, as well as the distribution of condoms and educational materials. They wish to expand their current services to reach the neighboring community (4,000 residents), add social media advertising and messaging to reach the younger population, distribute educational material and condoms to homeless shelters in the area, and add HIV testing to at-risk individuals. Select two of the potential services and determine how the money can be best spent to have the greatest outcome for the HIV+ population in the community.

  • Based on the CDC website, analyze how cultural norms impact the risk of getting HIV.  Explain one method or action on how to address this challenge.

Your initial contribution should be 250 to 300 words in length. Use proper APA formatting for in-text citations and references as outlined in the Ashford Writing Center.

Guided Response: Review several of your classmates’ posts.  Provide a substantive response (minimum of 100 words) to at least two of your peers. Imagine you are a client or resident within one of the communities served by this organization. Do you feel the grant funding has been spent wisely based on your peers’ selections for services and outcome of calculations? Make recommendations on at least one other service that could be included should the original option identified in your peer’s post be impossible to fulfill.

Part 1 of 2 – Part 1

This part of the question is based on the following information.

The Schmedley Discount Department Store has approximately 300 customers shopping in its store between 9 A.M. and 5 P.M. on Saturdays. In deciding how many cash registers to keep open each Saturday, Schmedley’s manager considers two factors: customer waiting time (and the associated waiting cost) and the service costs of employing additional checkout clerks. Checkout clerks are paid an average of $8 per hour. When only one is on duty, the waiting time per customer is about 10 minutes (or hour); when two clerks are on duty, the average checkout time is 6 minutes per person; 4 minutes when three clerks are working; and 3 minutes when four clerks are on duty.

Schmedley’s management has conducted customer satisfaction surveys and has been able to estimate that the store suffers approximately $10 in lost sales and goodwill for every hour of customer time spent waiting in checkout lines.

Please fill out the following table to answer the quiz questions in Part 1. (You may also download the attached worksheet.)

                                                           Number of checkout clerks

Part 2 of 2 – Part 2

This part of the question is based on the following information.

The wheat harvesting season in the American Midwest is short, and most farmers deliver their truckloads of wheat to a giant central storage bin within a two-week span. Because of this, wheat-filled trucks waiting to unload and return to the fields have been known to back up for a block at the receiving bin. The central bin is owned cooperatively, and it is to every farmer’s benefit to make the unloading/storage process as efficient as possible. The cost of grain deterioration caused by unloading delays, the cost of truck rental, and idle driver time are significant concerns to the cooperative members. Although farmers have difficulty quantifying crop damage, it is easy to assign a waiting and unloading cost for truck and driver of $18 per hour. The storage bin is open and operated 16 hours per day, 7 days per week, during the harvest season and is capable of unloading 35 trucks per hour according to an exponential distribution. Full trucks arrive all day long (during the hours the bin is open) at a rate of about 30 per hour, following a Poisson pattern.

The cooperative, as mentioned, uses the storage bin only two weeks per year. Farmers estimate that enlarging the bin would cut unloading costs by 50% next year. It will cost $9,000 to do so during the off season.

Discuss the most innovative idea you can dream up for a new business.  Explain why you think it would be successful.

Once I launch my event planning business and I get some events under my belt I would like to establish an application that potential customers can go to in order to view my previous events.  I would have them categorized by event type, budget, price point, etc.  This would give my potential clients an idea of the type of events I will put on, the costs associated by the type of event they want.  By doing this they would be able to compare my services with those of other vendors and choose mine due to the cost and outcome of the events.

Discuss the major challenges you would face in running such a business and how you would expect to overcome them.  Provide specific examples to support your response.

Some challenges that I foresee as I launch my business would be in getting my business name out there and establishing a reputation of being innovative, affordable and willing to work with any budget.  I’m a true believer that you can make an event nice at any budget and will provide my customers with examples of how we can make this work for them.  Based on my various contacts I would also be able to offer additional services.
